The integrated treatment you receive from your therapist is for your relief of pain associated with soft tissue and motor control dysfunction. You agree to immediately inform your therapist of any pain or discomfort so that adjustments to treatment protocol can be made. You understand that this treatment is not a substitute for medical examination, diagnosis, or treatment by a medical doctor. You understand that licensed massage therapists are not qualified to perform spinal or skeletal adjustments or to diagnose or prescribe treatment for any physical or mental illness, and that nothing said in the course of the session should be construed as such. Because treatment should not be performed under certain medical conditions, you affirm that you have stated all your known medical conditions and answered all questions honestly. You agree to keep your therapist updated as to any changes in your medical profile and understand that there shall be no liability on the therapist’s part should you fail to do so. By scheduling an appointment, you hereby agree to abide by the policies outlined above and you hereby waive your therapist, Claudia Gomez, from any and all liability past, present, and future relating to soft tissue manual therapy and bodywork. For minors under the age of 18, a parent or legal guardian must give consent for the minor to receive treatment.
